Ok Heroes... need some sleuthing and diggin' into the memory banks (and the interwebs) to list in this thread all known modules/racks that existed but no longer do. If you find a link or a picture that is even better...
I'll start out with the Aphex 4B, a 4 space aphex rack that might have quite possibly started the whole revolution.
As we gather info on these pieces I will start to add them to the database*
A few have already been listed here on LBH, I will dig around for those too.
(The 4B-1 is already in the DB.)
*currently shipping products will always take precedence.
Pico 500; now replaced with the FCS p3500.

Just posted for the "new materials for the data base" but here's a few you're missing.
Aengus EQ - 8 band graphic built for the oh-so-rare Aengus consoles (such as the one that used to be at Indigo Ranch). They have one of the desks for sale at sonic circus actually. Amazingly thick, powerful low end.
B&B EQF-2 - 3 band parametric plus sweepable high and lowpass filters. Kind of a funky design. Came with optional Jensen output tx.
Aphex EQF-2 - exactly the same as the B&B. I believe Aphex bought the company. Total steals if you can find them
APSI 559 - 9 band graphic with switches. Super aggressive upper mids. Awesome on guitars. Supposedly these were later rebranded and sold by API but I've never seen one of those.
APSI 561 - fixed 4 band program eq. I've never actually seen one
APSI 562 - 4 band sweepable parametric. What the Arsenal/JDK eq is based off of.
I have everything in my studio right now except the APSI 559 and a B&B branded EQF-2. I'll take some pictures when I get a minute and post them here!

Yes there are a number of legacy pieces not included in the DB yet but this is some good sleuthing! I have never seen hide nor hair of the Aengus, but was aware of their existence, just no info out there. Need photos!
Will get to these as soon as I can.

*edit - found photo of Aengus at Sonic Circus, will ask for permission to use it :-). Hmmm... I have seen those before but wasn't there another thumbwheel eq for the 500 series?

Dude. I bought 4. I had a lead on two more but cash was thin. Absolutely my favorite EQs I own.
I do have this picture that I'm using to sell a pair of 560s (btw). The Aengus' got caught offguard to the left.

I just designed something simple and am having front panel express make them up. I should get it in a week or two. If they work out I'll send you my fpx file.

Here ya go. Here's the 559 faceplates I made. I'm also putting up the front panel express file I used. In retrospect I should have used 2.5mm thick metal instead of 3mm. If you order some for yourself I highly recommend it. The LED buttons are kinda hard to push when it's too thick. I could have moved the numbering down a little too, right now it lines up with the top of each selection instead of the middle. They look awesome though if you ask me.

Those panels turned out nice. I discovered the same thing about the thickness of the panel when I made a front from an old blank panel. It needs to be thinner plus it needs a relief cut to clear the mounting tabs top & bottom. At the time, I only had hand tools but now that I have cnc, I might give it another go.

Probably not. There are some differences in some of the older modules. I think the 525s use the phantom power pin (15?) for link.

That photo is on Dan Alexander's website. I also have some old API literature from around that time (1980ish). I have an original 1604 color brochure which has a nice picture of a 575 next to a pair of 525s.

OK, I just noticed something else about this 1604. It appears to have 550A-1 EQs. I have one of these, and it has silver frequency knobs with blue inserts as opposed to the blue knobs on at least the newer 550As and Bs. I don't know if the knob color changed one day or if this distinguishes the A-1. It certainly doesn't say it's an A-1 on the front panel.
